In a large pot or Dutch oven, melt 2 tablespoons of butter over medium heat. Add the thinly sliced red onion and 1 teaspoon of salt. Sauté until the onions are translucent and starting to soften, about 5-7 minutes.

Add 2 tablespoons of water to the pot, scraping up any browned bits from the bottom. Let the water evaporate completely. Repeat this step two more times (for a total of three additions of 2 tablespoons water), allowing the onions to caramelize and deepen in color with each repetition. This process should take about 10-15 minutes.

Add the 1 pound of hot Italian sausage (casings removed) to the pot with the caramelized onions. Break up the sausage with a spoon and cook until it is browned and cooked through, about 5-7 minutes.

Add the 2 minced garlic cloves to the pot with the sausage and onions. Cook for about 1 minute until fragrant.

Pour 3 to 4 cups of beef broth into the pot. Break the 1 pound of spaghetti in half to fit into the pot, then add it to the simmering broth. Stir the spaghetti to ensure it is submerged in the liquid.

Bring the broth to a boil, then reduce heat to a simmer, cover, and cook until the pasta is al dente, stirring occasionally to prevent sticking, about 8-10 minutes.

Once the pasta is cooked, stir in the 1 1/2 cups of heavy cream.

Add the 6 ounces of grated Parmigiano Reggiano to the pot. Stir until the cheese is melted and the sauce is creamy and well combined.

Stir in 1 to 2 tablespoons of chili oil, adjusting to your preferred level of spice.

Serve the pasta immediately, garnished with fresh basil.
