Heat 1 tablespoon of olive oil in a large skillet or pan over medium-high heat.

Add the 1 pound of spicy Italian sausage to the pan. Break up the sausage into smaller pieces using a spoon or spatula while it cooks. Continue to cook until the sausage is browned and cooked through, about 6-8 minutes.

Remove the cooked sausage from the pan with a slotted spoon, leaving a small amount of the rendered fat in the pan. Set the sausage aside.

Add the 1/2 diced yellow onion to the same pan. Cook for 5 minutes, stirring occasionally, until softened and translucent.

Add the 4 minced garlic cloves to the pan. Cook for 1 minute, stirring constantly, until fragrant.

Pour in the 1/2 cup of vodka to deglaze the pan, scraping up any browned bits from the bottom with your spoon. Allow the vodka to cook for 2 minutes to allow the alcohol to evaporate.

Pour in the 1 cup of heavy cream and add the 1/2 teaspoon of red pepper flakes. Stir all the ingredients together to combine and bring to a gentle simmer.

Return the cooked sausage to the pan with the sauce.

Add the 12 ounces of cooked pasta to the pan. Pour in the 1/2 cup of pasta water and sprinkle in the 1/4 cup of grated Parmesan cheese.

Stir everything together thoroughly until the sauce becomes creamy and coats the pasta evenly. Cook for another 1-2 minutes until heated through.

Garnish with the 1/4 cup of fresh chopped parsley before serving. Serve immediately and enjoy!
