Prepare the mini meatballs: In a large bowl, combine the ground beef, 1/4 cup grated Parmesan cheese, bread crumbs, egg, milk, garlic powder, onion powder, dried oregano, 1/2 teaspoon salt, and 1/4 teaspoon black pepper. Mix gently with your hands until just combined, being careful not to overmix.

Simmer the sauce: Stir in the crushed tomatoes, tomato paste, dried basil, dried oregano, red pepper flakes (if using), and sugar. Season with 1/2 teaspoon salt and 1/4 teaspoon black pepper. Bring the sauce to a gentle simmer.

Add meatballs to sauce: Carefully add the browned mini meatballs back into the simmering sauce. Reduce the heat to low, cover, and let the sauce and meatballs simmer for at least 15-20 minutes, or up to 30 minutes, allowing the flavors to meld and the meatballs to cook through.

Cook the pasta: While the sauce is simmering, bring a large pot of salted water to a rolling boil. Add the spaghetti and cook according to package directions until al dente.

Combine and serve: Drain the pasta, reserving about 1/2 cup of the pasta water. Add the cooked pasta directly to the skillet with the sauce and meatballs. Toss to combine, adding a splash or two of reserved pasta water if the sauce seems too thick. Serve immediately, garnished with fresh chopped parsley and extra grated Parmesan cheese.
