In a large bowl, combine the ground beef, breadcrumbs, egg, minced garlic, onion powder, 1 teaspoon salt, 1 teaspoon black pepper, and 1 teaspoon Worcestershire sauce. Mix until just combined, being careful not to overmix.

Divide the mixture into 4 equal portions and shape each portion into an oval-shaped patty, about 1-inch thick.

Slowly pour in 1 1/2 cups of beef broth, whisking continuously to prevent lumps until the gravy is smooth.

Stir in 1/2 cup of heavy cream and 1 teaspoon of Worcestershire sauce. Season the gravy with salt and pepper to taste.

Bring the gravy to a gentle simmer and cook, stirring occasionally, until it thickens to your desired consistency, about 3-5 minutes.

Return the seared Salisbury steak patties to the simmering gravy. Reduce the heat to low, cover the skillet, and let the patties simmer in the gravy for 10-15 minutes, or until they are fully cooked through and tender.

Spoon the creamy onion gravy generously over the Salisbury steaks and serve hot with your favorite side dishes.
